So, in order to endure prompt and accurate processing of your paperwork, be sure to use this form, and NOT OLDER VERSIONS.
1. The updated instructions: the form handed in by the pupil's teacher is to be TYPED (See below for a link to download the form... be sure you save it on your computer).
2. A file with a request for English at level 3 that does not have the filled-in form of the teacher and/or a test sample (WITH WRITTEN WORK of the pupil - not multiple choice answers) - will not be discussed by the committee
